Benefits are available to all full-time employees starting on your first day of employment. Part-time and contract workers should check with HR for what applies.

Health insurance

We offer three medical plan options through [Carrier Name]:

Enroll or make changes within 30 days of your start date, or during the annual open enrollment window (typically November).

Dental and vision

  • Dental — $0/month for employee coverage, covers two cleanings and X-rays per year at 100%.
  • Vision — $0/month for employee coverage, annual eye exam covered, $XXX allowance for glasses or contacts.

401(k)

We match 4% of your salary up to the IRS annual limit. Vesting is immediate on matched contributions. Enroll through the HR portal. Changes take effect the following pay period.

Other perks

  • Home office stipend — $XXX one-time for new hires. See home office setup.
  • Learning & development — $XXX/year for courses, books, or conferences. Submit through the HR portal.
  • Wellness — $XX/month reimbursement for gym, fitness apps, or mental health services.
